  Foundational Community Supports (FCS) through Wellpoint is a program designed to help individuals with complex health needs achieve stability in housing and employment. By providing supportive housing services, FCS assists eligible individuals in finding and maintaining safe, independent living arrangements.

The program is available to Apple Health (Medicaid) beneficiaries who meet specific eligibility criteria, including those with behavioral or physical health challenges.

Supportive Housing Eligibility

To qualify, individuals must:

  • Be 18 or older.
  • Be Medicaid-eligible.
  • Have at least one health-related need, such as:
    • Mental health treatment.
    • Substance use disorder (SUD) treatment.
    • Assistance with multiple activities of daily living (ADLs).
    • A disabling health condition requiring long-term support.
  • Have at least one

  • Be 18 or older.
  • Be Medicaid-eligible.
  • Have at least one health-related need, such as:
    • Mental health treatment.
    • Substance use disorder (SUD) treatment.
    • Assistance with multiple activities of daily living (ADLs).
    • A disabling health condition requiring long-term support.
  • Have at least one risk factor, such as:
    • Chronic homelessness.
    • Frequent institutional stays (e.g., hospitals, psychiatric facilities, correctional institutions).
    • Frequent turnover of in-home caregivers.
    • A Predictive Risk Intelligence System (PRISM) score

The Foundational Community Supports (FCS) program application process involves several steps to ensure eligible individuals receive the right support. Here’s how it works:

  1. Determine Eligibility – Applicants must meet Medicaid eligibility requirements and have specific health-related needs or risk factors.
  2. Complete an Assessment – A contract

The Foundational Community Supports (FCS) program application process involves several steps to ensure eligible individuals receive the right support. Here’s how it works:

  1. Determine Eligibility – Applicants must meet Medicaid eligibility requirements and have specific health-related needs or risk factors.
  2. Complete an Assessment – A contracted FCS provider conducts an assessment to determine the level of support needed for supportive housing or supported employment services.
  3. Submit an Application – The provider submits the necessary forms and documentation to Wellpoint or the Washington State Health Care Authority (HCA).
  4. Approval & Enrollment – Once approved, individuals are enrolled in the program and connected with housing or employment support services.
  5. Ongoing Support – Enrollees receive continued assistance to maintain housing stability or employment success.
